Women, Infant, & Children

WIC provides nutritious foods which promote good health for pregnant women, women who have just had a baby, breastfeeding mothers, infants, and children up to age five. The program also provides nutrition and breastfeeding education and support.

Eligibility:

  • Women who are pregnant, breastfeeding, or have delivered a baby less than six months ago
  • Infants up to 12 months of age
  • Children up to the age of five.
  • All applicants must reside in Ohio
  • Participant must meet certain nutritional or health risk
  • Households must meet WIC income guidelines

What you need to bring to your WIC appointment:

  • Proof of income (pay check stub, Medicaid card)
  • Proof of address (Ohio driver’s license, utility bill, state ID)
  • Proof of identity (driver’s license, birth certificate, crib card, shot record)
  • All family members applying for WIC services
  • If pregnant, a doctor’s statement showing the due date
  • Children’s shot records
