WIC – WOMEN, INFANT & CHILDREN

WIC provides nutritious foods which promote good health for pregnant women, women who have just had a baby, breastfeeding mothers, infants, and children up to age five. The program also provides nutrition and breastfeeding education and support.
Eligibility:
- Women who are pregnant, breastfeeding, or have delivered a baby less than six months ago
- Infants up to 12 months of age
- Children up to the age of five.
- All applicants must reside in Ohio
- Participant must meet certain nutritional or health risk
- Households must meet WIC income guidelines
What you need to bring to your WIC appointment:
- Proof of income (pay check stub, Medicaid card)
- Proof of address (Ohio driver’s license, utility bill, state ID)
- Proof of identity (driver’s license, birth certificate, crib card, shot record)
- All family members applying for WIC services
- If pregnant, a doctor’s statement showing the due date
- Children’s shot records
